Georgia's 1 Star Nursing Homes are Everywhere

Every certified nursing home in the country gets an overall star rating from the Centers for Medicare and Medicaid Services: one to five stars, built from on-site inspections, staffing records, and clinical quality measures.

Across Georgia's 353 rated facilities the average is 2.68 stars. The national average is 3.0. Only three states sit lower: Missouri, Louisiana, and Illinois.

Thirty homes carry a federal warning beyond a low star rating. On the map, every one of them carries an outline.

Georgia's largest operator beats the average. Ethica Health operates 50 Georgia homes, the state's biggest portfolio, and averages 3.6 stars with just three one-star homes. Size is not the reason the others fail.

Four Questions to Ask

  1. How many minutes of registered-nurse time does a resident get here per day? At 39 Georgia homes it is under 15. Above 41 beats the national average.
  2. Who is on the floor on a Sunday night? Weekend RN coverage is in the federal record and it is often far thinner than the weekday tour suggests.
  3. What is your annual nursing turnover? Georgia averages 46 percent. A home well below that is keeping the people who know the residents.
  4. What company operates this home, and how do its other buildings rate? The operator's other buildings tell you more than the lobby does.
Chapter 3 -- Dig Deeper

The worst homes in Georgia share something: the company that runs them.

Seventy-four percent of Georgia's nursing homes belong to a multi-facility operator -- including 30 of the state's 48 five-star homes. What separates a safe home from a dangerous one is which chain runs it.

A handful of operators run portfolios that fail almost uniformly. Wellington Health Care Services averages 1.3 stars across its 10 Georgia homes; 6 of its 9 rated homes are one-star. Beacon Health Management: 1.4 across 10. At the other end of the size scale, Ethica Health runs 50 Georgia homes and averages 3.6 stars, with just 3 one-star homes -- proof that a large operator can run well. If you're touring one of the homes below, you're really touring all of them.

CMS 2026-06-01 Operator grouping uses CMS's own affiliated-entity (chain) field. Affiliation is self-reported and can trail an ownership change by a quarter or two.

Chapter 4 -- Federal Abuse Records

Twenty-two Georgia facilities carry a federal abuse warning. Eight more are under special scrutiny or named as candidates for it.

For some homes, the government has gone past a low rating to a formal warning. CMS marks a facility with an abuse icon when a recent inspection cited it for abuse, neglect, or mistreatment that harmed a resident or put one at risk. Twenty-two Georgia homes carry that icon today. When the harm behind that icon is fatal, the same federal record is what a wrongful-death claim is built on.

A separate program, Special Focus, singles out the homes with the most persistent problems for doubled inspections. Georgia has 2 Special Focus Facilities and 10 more named as candidates. Twenty-two homes carry the abuse icon; eight more carry a Special Focus designation without it, and four candidates carry both. Thirty distinct Georgia homes carry at least one federal warning. Across the last three inspection cycles Georgia's facilities drew 5,967 health citations, including 226 at immediate-jeopardy severity -- the level CMS uses when a deficiency has placed a resident in likelihood of serious harm or death -- and 165 more where inspectors found actual harm. Federal fines over the trailing three years total $4,996,514 across 134 facilities; the largest single-facility total is Nurse Care of Buckhead, at $277,501.

Methodology and sources

Data. Every figure on this page comes from the federal government's CMS Care Compare program (formerly Nursing Home Compare), processed 2026-06-01 and re-pulled 2026-07-07. The source datasets are the Provider Information file, the Health Deficiencies (citations) file, and the State and National Averages file, all published at data.cms.gov.

Ratings. Star ratings are CMS's. The overall rating combines health inspections, staffing, and clinical quality measures. Health-inspection ratings are assigned relative to other homes within the same state, so a Georgia three-star and an out-of-state three-star are not strictly comparable on that component. Three of the 356 facilities are too new to carry an overall rating and are shown as unrated.

Staffing minutes. "Minutes of RN care" is CMS's reported registered-nurse staffing hours per resident per day, converted to minutes. It measures registered-nurse time only, not the licensed practical nurse, aide, and therapist care that also reaches a resident. State and national figures are CMS's published averages.

Fines. The fine totals are federal civil monetary penalties in the CMS penalties file, which covers a trailing window of roughly three years (the Georgia penalty dates in this vintage run from June 2023 to March 2026). Older penalties age out of the file, so these are trailing-three-year totals, not lifetime totals.

Operators. Facilities are grouped by CMS's own affiliated-entity ("chain") identifier, not by fuzzy name matching. CMS affiliation is self-reported and can lag an ownership change by a quarter or two, so an operator's roster here may trail the current paperwork.

Special Focus. Special Focus Facilities and Special Focus candidates are two distinct CMS designations; this page keeps them separate rather than merging them into a single "special focus" count.
